OEM Part # BC3413005A. All main mounts in good condition. 

Small plastic guide pin and metal clip missing but not needed for installation (see yellow arrow). Exterior lens has normal wear and some minor cosmetic scratches (see yellow arrows). Scratches could be sanded and polished off. Back casing, plug and pins in good shape.
