This is a Hoover One Concept Upright Vacuum, Model U3335.  It comes with the converter attachment, hose, and hose attachment.  It has Power Drive, Height Adjustment, and Power Surge.  This vacuum is marked up but is in good working order as are the attachment set. It comes with 5 vacuum bags.

It has a shag, plus, and low/medium height attachment.  You do need to tilt the vacuum up to make the adjustment.  The hose attachment goes onto where the rbush is on the vacuum.
