The following are the condition guidelines I abide by(but I know it is subjective for each person). I will try to give an idea of the condition of Digipacks, but I will not typically rate Jewel Cases, as they are replaceable.

Mint (M): Disc looks new. Can be pre-owned but is mark free.

Near (NM): Disc is in near perfect condition; it can show handling marks/couple of small surface marks.

Very Good Plus (VG+): Disc will show more use and light surface scratches, but nothing deep or affect play.

Very Good (VG): Disc will have many light and deeper scratches and marks, but the disc will still play with no problems.

Good/Fair/Poor: I rarely have discs like this, as I personally avoid them.
